FMCH creates partnership with The Madison

FORT MADISON - Fort Madison Community Hospital Clinics is collaborating with The Madison, a Long-term Care Facility in Fort Madison to place an onsite Primary Care Provider to help manage their residents’ health care.

Family Nurse Practitioner, Michelle Hornung has assumed this role effective September 24, 2018.

Hornung has worked as a Registered Nurse (RN) at FMCH for the past 15 years. She recently completed her Masters in Nursing from Purdue University Global. Hornung holds a Bachelor of Science of Nursing degree from the University of Iowa and her Practical Nursing degree from Southeastern Community College.

As a Registered Nurse at FMCH, Hornung worked on the Medical/Surgical Unit, Special Care Unit, Emergency Room, the FMCH Clinics and the FMCH Wound Care/Occupational Health Clinic. She knew since she was five that she wanted to be a nurse.

“Even at a young age, I remember writing I wanted to be a nurse in a book by Dr. Seuss that asked ‘what do you want to be when you grow up’,” Hornung said.

“I made the decision to pursue my Masters in Nursing degree so I could make a difference by increasing access to health care for patients in our area. Working at The Madison allows me to act as an extension of the care the residents receive at FMCH. Being with them every day you become connected to them and they become a part of your family,” expressed Hornung. “I am looking forward to partnering with our Primary Care Providers at FMCH to ensure their health care needs are met.”

“We have been working with The Madison to find the right provider to place onsite for quite a while now. When Michelle achieved the status of Nurse Practitioner, we knew that with her background at FMCH she was the right person for the job. This is a model we would like to see replicated at other facilities in the area,” commented Matt Mullins, VP of Physician Services at FMCH.

Fort Madison Community Hospital Clinics employs twenty-six physicians and fourteen advanced practice professionals.
