County health teams seeking survey responses

LEE COUNTY - Lee County Health Department, Blessing Health Keokuk and Southeast Iowa Regional Medical Center invite you to be a part of the Community Health Needs Assessment (CHNA) process. To complete our county’s CHNA, we are collecting various county level data and also conducting a community-wide survey. The survey is being largely distributed to our public through organizational websites, social media, on-line survey monkeys, and by paper format available in key locations in the county. The survey is a great way to seek input from county residents on their views of what our top priority health needs or concerns are.

The survey will be open for completion through October 1, 2021.  We are asking all Lee County residents and those who work or receive health care services in Lee County complete the survey on-line or by paper format available at:

  • Lee County Health Department, #3 John Bennett Drive, Fort Madison
  • Blessing Health Keokuk, 1600 Morgan Street, Keokuk
  • Southeast Iowa Regional Medical Center, 5445 Ave. O, Fort Madison.

In addition, the three planning organizations will be hosting a virtual community townhall meeting in October 2021. This meeting will assess available county level data, analyze the community-wide survey results, and then identify 2-4 priority health needs to work on in the next few years. The group will also be asked to share their ideas for improvement strategies and activities they are willing to work on for improving the priority health needs.  Additional details as well as the date and time of the virtual community townhall will be coming soon.  If you have any questions about CHNA activities, please call one of the participating organizations and request to speak to someone involved in the CHNA planning process.
